- The details you give us when you book or fill in a form: your name, email, phone number and insurance information.
- Your dental and health history, x-rays and treatment notes from your visits.
- Basic technical information from your browser that keeps this site working, such as the pages you visit.
How we use it
- To schedule and care for you, and to send appointment confirmations and reminders.
- To check benefits and send claims to your insurance company.
- To run the practice, for example keeping accurate records and meeting our legal obligations.
What we don't do
We don't sell your information, and we don't share it with advertisers. We share health information only with the people who need it to treat you or process your claims, or when the law requires it.
Your choices
You can ask for a copy of your records, ask us to correct something that's wrong, or ask how your information has been shared. Ask at the front desk or email us and we'll take it from there.
